Description
This role is a full-time opportunity on a maximum term of two years based in Mount Isa.
About the role:
The Primary Health Care Nurse- Chronic Disease is responsible for;
- Providing comprehensive nursing care with a focus on chronic disease management; including health assessments, care planning and the delivery of preventative health outcomes
- Collaborating with multidisciplinary Health Care Teams both internal and external in chronic disease management
- Implementing and growing capacity of the new Primary Health Care Nurse- Chronic Disease role in Mount Isa
- Promoting local collaborations to ensure community engagement
The Mount Isa Primary Health Care Team provide essential healthcare clinics and services to rural and remote communities in North Western Queensland. Whilst the RFDS will always be known for its Aeromedical services, primary health care now makes up two-thirds of our daily operations, as a continued emphasis on prevention is vital to see better health outcomes for those that live in the bush.
From immunisations, health check-ups, referrals or management of chronic conditions such as diabetes, cardiovascular illness general lifestyle issues, RFDS primary health services provide regular medical assistance for people living and working remote Australia.
Who are we looking for?
You are an experienced and high-performing Registered Nurse with AHPRA registration, with exceptional patient care in the provision of chronic disease primary health care services with an equal commitment to contributing to team and operational goals.
In addition, you will have:
- Tertiary qualifications or post-registration qualifications in Nursing and / or Midwifery
- Post graduate qualifications in Chronic Disease Management (desirable)
- Demonstrated experience in postgraduate nursing practice in Chronic Disease
- Previous experience providing PHC in a multi-disciplinary team environment in a rural and remote setting
- Experience in Indigenous and rural and remote nursing practice
- Ability to travel in a light aircraft, have overnight stays in remote, rural and indigenous communities if needed.
- Experience in working in autonomous roles with minimal supervision
- Flexibility with working outside of core business hours
Nice to have;
- Current Advanced Life Support (ALS) and Paediatric Advanced Life Support (PALS) certification (Can be obtained post-employment offer within 90 days)
- Immunisation Endorsement with Queensland Health
- Scheduled Medicines Rural and Isolated practice (SMR and IP) endorsement

You will be required to successfully complete the RFDS credentialing process which includes providing copies of qualifications/registrations and evidence of immunity or vaccination against VPD.
