The Royal Flying Doctor Service South Eastern Section (RFDSSE) is part of one of Australia’s most respected and trusted organisations, providing 24-hour emergency aeromedical retrievals and essential healthcare to people living and working in remote, rural, and regional communities. As the contracted provider for the Western NSW Primary Health Network (PHN), RFDSSE delivers the Keeping Healthy Puura Manti Program (KHPMP)—a time limited initiative designed to improve chronic disease outcomes across the region.
The program strengthens access to culturally appropriate, integrated care by working alongside primary care providers, allied health, and community services to enhance health literacy, self-management, and overall wellbeing.
About the Opportunity:
The Care Support Navigator is a key member of the KHPMP team, supporting clients with chronic disease to access coordinated, culturally safe care. This role facilitates seamless transitions across primary care, allied health, and hospital services, addressing non-clinical barriers to care. Using a strengths-based and culturally responsive approach, the Care Navigator empowers clients and families to understand their health needs, navigate the healthcare system confidently, and experience a more coordinated, holistic health journey.
Key Responsibilities:
- Support clients to understand and engage with their healthcare plans, referrals and follow-up appointments, helping remove barriers such as transport, financial or social needs.
- Coordinate care in collaboration with GPs, Nurses, Allied Health Workers, Allied Health staff and external services to ensure timely, integrated support.
- Assist clients during transitions of care, including hospital admission and discharge, ensuring continuity and reducing gaps in care.
- Advocate for culturally safe, respectful and appropriate care across all services, promoting client trust and confidence.
- Build strong, trusting relationships with clients, families, and communities through culturally responsive engagement.
- Provide culturally appropriate health information and support to promote wellbeing, health literacy, and self-determination.
- Monitor appointments, referrals, and follow-ups to maintain continuity of care.
- Work effectively within a multidisciplinary team, maintaining clear, respectful, and timely communication with clients and health professionals.
- Maintain accurate, confidential records in accordance with AHPRA, legislation and RFDS policies.
- Participate in ongoing professional development and contribute to community health education initiatives when required.
Essential Criteria:
- Relevant qualifications in Nursing (Registered or Enrolled), Social Work, or Aboriginal Health Practice (Cert IV/Diploma)
- Experience working in rural or remote communities
- Experience in health, community or social services, care coordination or client navigation
- Understanding of primary healthcare, referral pathways, chronic disease management and social determinants of health
- Strong communication, organisation and multitasking skills
- Ability to build rapport with clients and work effectively within a multidisciplinary team
- Ability to maintain accurate documentation and manage competing priorities
Mandatory Requirements:
- NSW Working with Children Check (or ability to obtain)
- Category A vaccinations
- National Police Check
