We may have collected personal information about you (such as your name and contact details) from another person who has provided your information to us. Otherwise we will collect information from you directly. We may also collect information about you from other service providers or the public record.

We collect your details for the purposes of receiving donations from you or someone on your behalf as a donor, and to contact you regarding your donation and possible future donations. We also collect information so we can contact you about the services we offer, events, to deal with complaints or stakeholder management, and to comply with laws.

We may use your personal information for quality improvement and education purposes. If you do not complete all of the information in this form or provide accurate information, we may not be able to adequately address any issues raised or make improvements, give you updates about how we have addressed the feedback, or contact you further.
