The Royal Flying Doctor Service South Eastern Section (RFDSSE) is seeking a passionate and experienced Virtual Diabetes Educator to join our Keeping Healthy Puura Manti Program (KHPMP). In this rewarding role, you'll provide evidence-based diabetes education and chronic disease management support through telehealth and digitally enabled care, helping improve the lives of people living with diabetes, pre-diabetes and other chronic health conditions across Far West NSW.
About the Role
As a Diabetes Educator, you will work as part of a multidisciplinary team delivering accessible, person-centred and culturally safe care to individuals and communities across rural and remote Australia. Using telehealth platforms and digital health technologies, you will empower clients to better understand and manage their diabetes while supporting prevention, early intervention and improved long-term health outcomes.
Key Responsibilities
- Deliver individual diabetes education and self-management support via telehealth.
- Conduct comprehensive diabetes assessments and care reviews.
- Support clients with blood glucose monitoring, diabetestechnologies and medication management within your scope of practice.
- Develop andmonitorpersonalised care plans in collaboration with clients,families and healthcare providers.
- Facilitate virtual education sessions,workshops and group programs.
- Work closely with GPs, Aboriginal Health Practitioners, nurses, dietitians, pharmacists and allied health professionals.
About You
To be successful, you will bring:
- Qualification in a health profession recognised for diabetes education (such as Nursing, Dietetics, Pharmacy or another ADEA-recognised discipline).
- Postgraduate qualification in Diabetes Education and Management or equivalent.
- Current Credentialled Diabetes Educator (CDE) status or demonstrated progression towards credentialling.
- Current professional registration or membership relevant to your discipline.
- Experience delivering diabetes education and chronic disease management services.
- Strong communication skills with the ability to engage clients through virtual care platforms.
- Experience working collaboratively within multidisciplinary healthcare teams.
- Competency using electronic medical records and digital health systems.
Highly Desirable
- Experience providing telehealth services.
- Experience working with Aboriginal and Torres Strait Islander communities.
- Experience in rural,remote or community-based healthcare.
- Previous involvement in group education programs, quality improvement or chronic disease initiatives.
