Broken Hill – Full Time Contract until 30 June 2023
The Royal Flying Doctor Service South Eastern Section (RFDSSE) is seeking an experienced nurse to work as a Practice Nurse in our Clive Bishop Medical Centre (CBMC) Primary Health Team.
This role will act as the care navigator providing community and social coordination services as part of the Far West Local Health District Integrated Care Staying Healthy Program. The program is focused on patients who have been identified or at risk of developing a lifestyle related chronic disease.
Your role will involve providing support, advocacy, information and access to community resources to assist patients to make changes to their lifestyle behaviours to improve and maintain their health status.

The ideal candidate for this position should have significant experience in general primary health with chronic disease management, an immunisation certificate and clinic experience.
To be successful in this role you will require:
- Registered Nurse with non-conditional AHPRA registration;
- Current Drivers Licence;
- Demonstrate a high level of written and verbal communications skills;
- Be able to prioritise your work and manage time effectively;
- Demonstrated ability to work with challenging consumers, families and service providers;
- Demonstrated knowledge and experience in computer skills including word processing and database.
- Evidence of full COVID-19 Vaccination.
