Description
The Royal Flying Doctor Service of Australia (RFDS) is one of the largest and most comprehensive aeromedical organisations in the world. Using the latest in aviation, medical and communications technology, we deliver telehealth and a 24-hour emergency retrieval service to rural and remote Australia.
The Royal Flying Doctor Service of Australia (South Eastern Section) (RFDSSE) service has successfully tendered to deliver a 3-year Chronic Disease Management Program for sites across the Far West. We are looking for a Program Lead who is self-motivated, has well developed written and verbal communications skills, good time management and organisational capabilities and is able to demonstrate attention to detail to join our team in Broken Hill.
As the Program Lead you will be responsible for the day-to-day coordination of the program operations and leading a team. You will develop, implement and coordinate a primary health chronic disease management program and provide care navigation needs to consumers engaging in the program.
Responsibilities also include:
- Develop a plan for stakeholder identification, engagement and communications
- Develop a gap analysis and needs assessment based on available data and stakeholder needs
- Review the project KPIs and develop a program to deliver KPIs
- Ensure evaluation is undertaken throughout the program and data is collected for reporting
- Provide coordination of care to patients who participate in the program, including regularly reviewing patients to ensure the program is meeting their care needs
- Provide suggested referral pathways for allied health services to patient’s usual GP
- Ensure accurate and timely reports are prepared for the RFDSSE Health Leadership team, Board, and PHN to demonstrate progress towards program KPIs
- Adopt a continuous improvement approach
- Ability to manage confidential information appropriately.
- An understanding of rural and remote practice in the context of Primary Health Services
- Demonstrated skills in leadership
- A knowledge of quality improvement systems
- Excellent verbal and written communication skills including ability to produce resources, presentations, reports, literature reviews, fact sheets, and correspondence in a clear and concise manner
- Demonstrated high level organisational and team skills including the ability to prioritise, work on a variety of activities simultaneously, maintain attention to detail and deliver high quality work to agreed deadlines
- Experience working in health-related area
