Description

The Royal Flying Doctor Service South Eastern Section (RFDSSE) is seeking an experienced and energetic person to work as part of our Clive Bishop Medical Centre (CBMC) Primary Health Team.

This role is responsible for the provision of community and social coordination services as part of the Far West Local Health District Integrated Care Staying Healthy Program. The program is focused on patients who have been identified or at risk of developing a lifestyle related chronic disease.

Your role will involve providing support, advocacy, information and access to community resources to assist patients to make changes to their lifestyle behaviours to improve and maintain their health status.

To be successful in this role you will require:

  • Relevant qualifications with a minimum Certificate IV in a health or social and welfare service related field or be willing to work towards;
  • A Current Drivers Licence;
  • Demonstrate a high level of written and verbal communications skills;
  • Be able to prioritise your work and manage time effectively;
  • Demonstrated ability to work with challenging consumers, families and service providers;
  • Demonstrated knowledge and experience in computer skills including word processing, database.
Knowledge of local services, and recent experience in a similar role is desirable.
