We are seeking motivated and experienced nurses in the area of Chronic Disease management and/or Community Midwifery.

The Chronic Disease Coordinator will be required to effectively support the identification of patients with high coordination and team needs and enhance access to holistic coordinated care. For the position description please click here.

The Community Midwife will be required to provide high quality, best practice Maternal and Child Health services to women and their families who live in the remote and isolated areas of South Australia on a share care model where no midwife is currently employed.
