About this Opportunity
Based out of Port Augusta, the Community Health Nurse – Chronic Disease effectively supports the identification of patients with complex care and enhances access to holistic coordinated care with the aim to achieve better health outcomes for the people who live in the remote and isolated areas of the far North and West of South Australia.
About You
You have a passion for Primary Health Care with a strong interest in community-based nursing including patient education and health promotion. With a minimum of five years nursing experience, ideally you have postgraduate nursing qualifications in Chronic Disease Management or similar with strong clinical skills and experience working with rural and remote communities. Excellent communication skills and a desire to work collaboratively in a multi-disciplinary team are essential.
