About this Opportunity
As the Community Health Nurse – Chronic Disease/ Diabetes Educator, you will work closely with the existing Primary Health Care team to deliver comprehensive diabetes self-management programs in both a group and one-on-one setting, empowering people to live well with chronic conditions/diabetes and raising awareness for those at risk in rural and remote South Australia.
The diabetes educator will have experience in a clinical or community setting providing education, information, and support in both a group and one-to-one situation, as well as experience in liaising with general practitioners and health professionals. Participation in our annual Youth Camps is a necessity, as well as experience in the development and delivery of health programs.

Specifically, you will:
- Coordinate care for patients with Chronic Disease and Complex Care Needs
- Competently perform clinical nursing duties required in the preparation, execution and monitoring of care plans
- Support patients in learning more about their conditions and self-management
- Connect patients to appropriate specialists, allied health and support services.
- Maintain accurate and legally acceptable patient records, ensuring the security of patient documentation
About You
You are:
- An experienced registered Nurse with an interest in community-based nursing, patient education, health promotion and a commitment to continuing professional development
- An excellent communicator, with a highly developed interpersonal skills and the ability to build collaborative working relationships with internal and external stakeholders.
- Understanding of cultural sensitivity and its application in healthcare
- An extremely organised individual with well-developed planning skills
- Able to work effectively within a multi-disciplined team while demonstrating flexibility, adaptability, and initiative.
To be considered for the role you require:
- General nursing registration with AHPRA
- Relevant tertiary qualifications, including post-graduate studies in the field of diabetes education and management or other chronic conditions related field.
- Experience in diabetes/Chronic conditions education at a clinical or community level.
- Demonstrated experience in the delivery of health education either in a one-to-one or group setting.
- Minimum of five years nursing experience, preferably in a community-based role.
- Demonstrated understanding of the needs of Primary Health Care and interest in community-based nursing.
- Current and valid driver’s licence issued from an Australian State or Territory
- Knowledge of the goals of diabetes/chronic conditions management in particular Type1/Type 2 Diabetes Mellitus, Gestational Diabetes, Chronic Kidney Disease, Asthma and COPD.
