About this Opportunity
You will join other dedicated health professionals as a member of our multidisciplinary Primary Health Care Team, delivering health services via road or air to rural and remote locations. As an experienced community-based Nurse, you have a passion for patient education and health promotion. Your strength lies in your ability to facilitate excellent patient care through the development of care plans and connections to specialists, allied health and support services.
About you
You will be an AHPRA Registered General Nurse, with a minimum of 5 years nursing experience, preferably in a community-based role, with a professional approach to service delivery. You will possess an understanding of rural and remote health care, with the ability to reinforce our values and enjoy working collaboratively with other health care professionals. A Postgraduate qualification in Chronic Disease Management or the like is highly regarded.
