About this Opportunity


You will join other dedicated health professionals as a member of our multidisciplinary Primary Health Care Team, delivering health services via road or air to rural and remote locations. As an experienced community-based nurse, you have a passion for patient education and health promotion. Your strength lies in your ability to facilitate excellent patient care through the development of care plans and connections to specialists, allied health and support services.

To be successful in this role you will have:

  • General nursing registration with AHPRA
  • Minimum of five years nursing experience, preferably in a community-based role
  • Demonstrated understanding of rural and remote primary health care
  • Ability to engage with a wide range of primary health stakeholders
  • Postgraduate qualification in Chronic Disease Management or the like is highly regarded
