The Royal Flying Doctor Service (South Eastern Section) is now looking for a suitably experienced Endorsed Enrolled Nurse or Registered Nurse to join the team at Clive Bishop Medical Centre at Broken Hill in the role of Practice Nurse – Chronic Disease.
This role is responsible for providing comprehensive health care in Broken Hill and supporting our attending network patients. The role encompasses clinical, quality and educational services across the age spectrum.
Responsibilities include but not limited to:

  • Undertaking of chronic disease management Medicare item number activities, preparation of GP Management Plans and team care arrangements and reviews
  • Completion of Health Assessments including Diabetes Risk Assessment, over 75 Health Assessment and 45–49-year-old Health Assessment
  • Development of Care Plans under Coordinated Veterans’ Care Program, including working with patient’s usual GP, identification of eligible patients, and development of recall system
  • Using data extraction tools such as PenCAT to identify list of eligible patients for chronic disease item numbers and health assessments
  • General Practice nursing duties
To be successful in this role you will require:
  • Endorsed Enrolled Nurse or Registered Nurse with non-conditional AHPRA registration
  • Current Drivers Licence
  • Demonstrate a high level of written and verbal communications skills
  • Be able to prioritise your work and manage time effectively
  • Excellent computer literacy, prioritisation and time management skills
  • Ability to work in a multi-disciplinary team
  • Collaborative approach to problem solving
  • Evidence of full COVID-19 vaccination
