Difficult Airway Techniques

Difficult airways pose a significant challenge in medical retrieval, particularly in rural and remote settings. Some useful links to training videos on clinically relevant topics are listed here.

 

Cannula & scalpel cricothyroidotomy or tracheotomy techniques

The following techniques have been developed and recorded by Dr AMB Heard and colleagues in the Department of Anaesthesia, Royal Perth Hospital.  They cover an algorithm and techniques for the 'Can't Intubate Can't Ventilate' scenario.  Click on the links below to go directly to the videos in YouTube format.        

Cannula cricothyroidotomy

A detailed description of how to safely and successfully insert a cannula through the cricothyroid membrane or trachea for emergency jet ventilation. Also how to quickly ensure correct placement. This is the first step for every case of can't intubate can't ventilate in flow chart shown.

Melker cricothyroidotomy

Description of how to convert a cannula emergency airway into a definitive airway. The authors do not recommend the Melker as a first line technique, but the only Seldinger cuffed airway that can be recommend for securing the airway.

Scalpel finger cannula technique  

A Technique for managing the can't intubate can't ventilate scenario in a patient with no or little palpable neck airway anatomy. Assumesinitial percutaneous cannula attempts have failed.

Scalpel bougie technique  

Surgical Technique for securing an emergency airway, aimed at anaesthetisist with little surgical experience and hence little exposure to scalpel use. Avoids any scalpel manipulation and is safe, simple and fast. Developed By Dr A Heard, Royal Perth Hospital Western Australia.

A good demonstration of the Fastrach intubating LMA device and the included silicon-tipped wire-reinforced endotracheal tube.

Self intubation with the intubating LMA  

An interesting demonstration of awake intubation with an intubating LMA.  Dr Chandy Verghese demonstrates the awake insertion of an iLMA® (intubating laryngeal mask airway) in himself after local anaesthetic spray and injection.  The iLMA® is known as the LMA-Fastrach™ outside of the United Kingdom.
